CGHS beneficiaries asked to apply for plastic cards
Hyderabad, June 24 (INN): Additional Director of Central Government Health Scheme (GGHS) A. Vijaya Kumar informed that the existing pensioners/service CGHS card beneficiaries who have not applied for plastic cards have to submit required documents at their wellness centers before June 30.
These forms can be downloaded from www.cghs.nic.in. First click on apply online plastic cards (Existing beneficiaries-click existing card number/ beneficiary ID) download the application and submit in the concerned wellness centers along with necessary enclosures along with two sets of photos of individuals, Aadhar card copy of all the members, nomination from, existing CGHS card copy, certificate of physically handicapped/supporting documents for dependents.
Serving beneficiaries should submit the application for plastic cards through their department with the all enclosures to the Additional Director's office before June 30, added Vijaya Kumar.
